Initial Consultation

With our comprehensive, 5-step process, we deliver care that’s perfectly customized for the needs of your loved one. Curious to learn more? Here’s a bit of insight into our process!

Initial consultation – During our initial consultation, we’ll discuss a wide variety of issues, in order to begin developing an initial homecare plan. We’ll help guide you through the process, and discuss things such as:

  • Disabilities and health conditions that may require medical assistance
  • Physical needs (bathing, hygiene, diet, mobility, incontinence, etc.)
  • Expected schedules (sleep, medicine, meals, errands)
  • Medical needs (doctor visits, medication schedules, health levels)
  • Specific home assistance needs (meal preparation, medical care, laundry, housekeeping)
  • Emotional needs (companionship, sociability, regular social events such as church attendance)
  • Current level of in-home care
  • Expected budgetary requirements and specific healthcare waivers/assistance eligibility.

During this initial consultation, we’ll begin developing a comprehensive care plan, based on the information that you provide.

Home assessment – Our home assessment allows our staff to understand the current level of care being provided to your loved one, and understand the things necessary for them to stay independent during long-term in-home care.

This assessment is very useful in determining what services your loved one needs. Rather than comprehensive care, for example, they may simply require some basic assistance with errands and meal preparation, and daily companionship.

We can also provide expert recommendations for home modifications, such as the addition of chairlifts or guardrails, and safety precautions such as bathroom guardrails. After our home assessment, we use the insights gained from our consultation and assessment process to develop a comprehensive plan and care schedule.

Scheduling

Scheduling/initial plan development – With your approval, we’ll devise a comprehensive schedule and care plan for your loved one. Whether you’re looking for long-term care or just shorter-term respite care, we’ll create a plan that’s uniquely suited to the ability levels, requirements, and living situation of your loved one.

After our plan has been developed and meets with your approval, we’ll begin delivering the services you requested, as outlined in your planning documents.

Care program implementation – After you’ve chosen Agua Cristalina Home Care, LLC., we’ll implement our customized care plan, and begin assisting your loved ones with their daily needs. We’ll schedule regular check-ins and communication calls to keep you informed about the health and well-being of your loved ones, and discuss how they’re adjusting to their new in-home care program.

Continuous improvement – As we continue to provide care for your loved ones, we’ll gain a deeper understanding of their needs and their personal requirements. By doing so, we can get a more complete perspective, and adjust our caregiving accordingly.

We’ll stay in constant contact with you about potential recommended changes in care. For example, if we find that your loved one’s physical health is deteriorating, or that they may require specialized help for a condition like dementia, we can recommend the appropriate services, and ensure that they have all of the help they need to maintain their independence.
